Student Survey

The Center for School Success (CSS) is conducting a survey to determine in what ways families benefit the most from CSS assessment services. CSS conducted a similar survey in 2005 which provided valuable feedback as to how CSS can better meet the needs of its assessment clients.

We greatly appreciate your participation. We hope to have all survey's completed by October 30, 2008.  The survey is mostly fill-in-the-blank format with opportunities for comments. It should take approximately 15-20 minutes to complete.

For those of you with middle/high school students we are enclosing a one page survey for your child to complete. Their responses are very important to us. Please encourage them to complete it and return it in the self-addressed envelope. We will enter all students’ names who return the survey into a raffle for a $25 iTunes Gift Card.

Please make sure you have time to complete the entire survey as there is not an option to save answers and come back at a later time. Feel free to review the questions before completing the survey.
